Description: Discover effective strategies for managing the transition to adulthood for patients with 22q11.2 deletion syndrome (22q11.2ds), a genetic disorder affecting one in every 4,000 individuals. The transition to adulthood poses unique challenges, necessitating specialized coordination of care. This session will address the complex, coordinated care needs of these patients, focusing on neuropsychiatric challenges, the role of multidisciplinary teams, and potential transition models. Learners will gain insight into how 22q11.2ds clinics can play a vital role in improving healthcare outcomes during this critical period.
Learning Objectives:
At the completion of this activity, learners will be able to:
- Describe the common active concerns and problems associated with 22q11.2ds in adolescence and young adulthood.
- Identify the evolving healthcare needs of patients with 22q11.2ds throughout different stages of life.
- Describe care strategies that utilize a multidisciplinary team approach to better equip patients with 22q11.2ds as they transition into adulthood.
Learning Outcomes:
At the completion of this activity, learners will be able to:
- Self-assess a gain in knowledge related to improving transitions of care in adolescent patients with 22q11.2 deletion syndrome.
